Job Title: Community Nurse – Whānau Health & Home Support Assessments
Location: Whaioranga Trust – 531 Welcome Bay Road, Tauranga
Reports To: Manager
Hours: 30 hours per week
Remuneration: $36-$45 per hour (dependant on experience), Support with Further Study!
Employment Type: Part-time Permanent
Purpose of the Role
To provide culturally responsive nursing through the assessment of home-based support referrals for age-related disabilities and the delivery of community-based health checks. This role prioritises equitable outcomes for whānau Māori and ensures that services are delivered in a manner that upholds mana, fosters whanaungatanga, and supports holistic wellbeing.
Key Responsibilities
- Home-Based Support Assessments – completing clinical assessments in disabilities sector to assess for home based support services
- Community-Based Health Checks – conduct health and wellbeing assessments for whanau, screen for long-term conditions, actively target and engage with whānau Māori, ensuring an increase in accessibility
- Clinical Guidance & Support – provide clinical guidance to teams, ensuring practice aligns with clinical and cultural safety, strengthen quality improvement and care delivery
Qualifications and Experience
- Registered Nurse with current APC (NZNC)
- Minimum 2 years’ experience in community, primary care, or aged care nursing
- Experience working with Māori communities and understanding of tikanga Māori preferred
- Experience in team-based or supervisory roles an advantage
- Knowledge of home-based support services and NASC assessment processes an advantage
- Current NZ driver’s licence
- Completed or looking to study towards a post graduate certificate in nursing

- Ability to lead and support others clinically

- Excellent communication and documentation skills
- Able to work independently and as part of a kaupapa Māori health team
- Confident using digital health systems (e.g. InterRAI and other CMS systems)
